Primary Briefing
Bleed represents an open that continues to damage a creature as it exerts itself.
Bleed has two values: Potency (the damage dealt each time the bleed triggers), and Count (the number of times the bleed can trigger before ending).
A creature suffering from Bleed triggers it whenever it exerts itself, including when it makes an attack, takes an Action, takes a Bonus Action, uses a Reaction, or moves.
When Bleed triggers the creature takes damage equal to the Bleed’s Potency, and the Bleed’s Count decreases by 1. When the Count reaches 0, the Bleed ends.
If a creature already suffering from Bleed gains Bleed again, increase either the Potency or the Count of the existing Bleed, as specified by the effect that applied it. Bleed is tracked as a single effect with combined Potency and Count.
Bleed ends when its Count reaches 0. A creature can also attempt to staunch the wound. As an Action, a creature can make a DC 15 Wisdom (Medicine) check to end Bleed on itself or a creature within reach. Bleed also ends if the creature regains any hit points, whether from magical or nonmagical healing.
